Ways to donate ....

Many patients wish to remain in their own home, close to family and loved ones, for as long as possible. To ensure this can happen, the Foyle Hospice Community Palliative Care Team provides professional palliative care expertise and support to patients and families in their own homes.

Our Community Palliative Care Team works closely with the patients GP and District Nurse to provide support and advice to ensure that patients receive the best quality of care available.

The role extends to meet the social and emotional needs of patients and families.

The Community Palliative Care Team is closely supported by the medical team within the Hospice and other health care professionals
